Orders may be placed via our website, by fax, or calling our offices directly at (937) 236-3182. Call us and we will have a custom FAX ordering form sent to your company. Back Orders-Our fill rates are among the highest in the industry. However, we occasionally experience out-of-stocks on some merchandise. Superior Dental & Medical Supply will not be responsible for any inconvience caused by a back order. We will attempt to notify you in regards to product backorders. Our website does have a $10.00 minimum order amount. If you call our office to place an order less than $10.00 then you will be charged an additional $4.50 fee on your order in addition to regular shipping charges.
Our Return Policy
If you are unhappy with your order for any reason, please return products in their original condition and packaging, with the invoice within 30 days from the date of your order. A refund of the product's purchase price will be credited to your credit card account once we have received and processed the returned product(s), less the original shipping charge, in some cases credits may take up to 14 days to process. A handling fee or restocking fee may be applied. How to Return: Mail the product in the original packaging to our return center. Please send the invoice with the product. Circle the item(s) you are returning on the invoice. If a product was sent to you in error, we will provide you with a pre-paid shipping label for the return. To return your order, simply reseal the original shipping box if possible, and contact customer service at (937) 236-3182, or e-mail them at sales@superiordentalsupply.com We will work with you to make arrangements for returning your order. You are responsible for the cost of shipping your return to us. All returns must be returned in original packaging and are subject to inspection by Superior Dental and Medical Supply. No credit will be issued for product we determine cannot be resold. Any product shortage, damage or shipping errors must be reported within 48 hours from receipt of the order. If we shipped you the wrong product in error, then we will pay the full cost of returning the product back to us. If you open or use the product then we cannot take the product back for credit. The customer will be responsible for the shipping charges and handling fees when orders are cancelled after they have been shipped.
Our Shipping Policy
Approximate Charges for shipping are calculated at time of order. Orders placed before 1:00 p.m. EST (12:00 p.m. in all other time zones) will be shipped the same day. Additional shipping charges may be applied on shipments with incorrect addresses, and orders less than $10.00. Orders less than $10.00 may be charged an additional minimum order fee of $4.50. A $10.00 fee will be applied to any order once shipped and then refused by recipient. On some items we do offer free shipping however we do reserve the right under certain circumstances to not offer free shipping on free shipping items the customer would be notified of this. Occasionaly actual shipping charges may differ depending upon the quanity and size (boxes) of your order the customer will be notified of the difference in shipping charges and upon confirmation the order will then be shipped out.

Our Terms and Conditions
The Superior Dental & Medical Supply Terms and Conditions of sale of its products are as set forth in its Credit Application. Standard terms are net 30 days from invoice date after Credit Application has been approved. Payment options include Visa, Mastercard, money order, personal check with approved Credit Application.
Pricing- We use a variety of ways to promote our website. We are not responsible for incorrect pricing found at places other than directly on our website. Many search engines, affiliates and shopping portals do not always update their content on a timely basis. The prices found directly on our website will always be honored. Ross products are only available to Authorized Health Care Providers for own use. "Own Use" shall mean those uses that are part of providing health care services and / or products directly to patients who are in an institutional or home health care setting within the fifty (50) United States, "Authorized Health Care Provider" shall mean a person or entity that; (a) is licensed to provide health care services and/or products directly to patients in an institutional setting (b) provides health care services and/or products directly to patients in an institutional setting as an agent of a third party payor such as Medicare or Medicaid, or (c) provides health care services and/or products directly to its patients in a home health care setting and such patients are under the care of a licensed health care professional and have received a written order issued by such professional for use of Products under such professional's plan of care, and such person of entity has been designated in writing as the authorized agent to supply Products pursuant to such plan of care.
